Making Office Appointments

When you call to make an appointment, we can help you best and most efficiently when we have the following information ready. You will talk to our concerned call specialists, who will ask you some questions to make sure you are seen by a clinician on a schedule that best suit your needs.

  • A list of your current symptoms, complaints, or needs and prior or current health diagnoses or problems, to determine what kind of appointment is needed
  • A list of your current medications and dosages
  • Your insurance information, date of birth, and social security number (for insurance)
  • When you are available for an appointment and current phone number
  • If you need transportation assistance

On your first visit with us,

  •  Arrive 15 minutes early for the appointment to give us time to prepare your Patient records
  • Bring your completed new patient papers
  • Bring your insurance card and all current medications with dosages

Our call specialists and triage nurses may need more information as they talk with you about your health care needs.
